I know we can't put too much credence on a recall study, but hopefully this will lead somewhere interesting.

Possibly, McCullough said, it wasn't the bread consumption, per se, that raised the risk of cancer, but something related to it, such as eating a lot of butter with the bread.
Of course!!! It's the fat!!! Speaking like a true dietitian!
Actually I agree with McCullough about the limitations of the study and the conclusions being a bit rushed when other sugar and starch-containing foods didn't show the same increase in risk. But to stretch it and say that it is because the bread may have been consumed with butter is just as wrongly interpreted as the authors of the study.
A plausible link between the amount of glucose and kidney damage may be the increase in what's known as AGE prducts (Advanced Glycation-End products), which have been shown to damage cells due to increased amount of intracellular glucose that may bind unspecifically to proteins making them dysfunctional.

I think the Warburg hypothese has also something to do with it. If you do not know about it, here some links to articles making the connection between energy metabolism and cancer.
